
HappyTimes Agreement
Please read the following agreement and fill out the form below.
Services Provided
HappyTimes agrees to board the Owner's dog from the boarding start date to the end date, during which time the dog will receive care, feeding, exercise, and attention in accordance with the dog’s needs.
Owner Responsibilities
Owner certifies that the dog is in good health, free from any infectious or contagious diseases, and is current on all vaccinations, including rabies, Bordetella, DHPP, distemper, parvovirus, and is on monthly prevention for fleas, ticks, and heartworm. Proof of vaccination must be provided prior to boarding.
Owner must disclose any medical conditions or allergies the dog may have and provide any necessary medication with clear instructions.
Dog’s Behavior
Owner certifies that the dog has no history of aggressive behavior towards other animals or humans. If the dog displays aggressive or uncontrollable behavior, HappyTimes reserves the right to contact the Owner or emergency contacts provided for immediate pickup.
Temperament Testing
HappyTimes requires a temperament test to assess the dog’s behavior prior to boarding and daycare. If the dog shows signs of aggression, severe anxiety, illness, or any other behavior that could pose a risk to staff or other animals, HappyTimes reserves the right to refuse service or request immediate pickup.
Emergency Care
In the event of a medical emergency, HappyTimes will attempt to contact the Owner or the emergency contact listed. If the Owner cannot be reached, HappyTimes is authorized to seek veterinary care for the dog at the Owner’s expense.
Dog’s Personal Items
Owners should not bring bedding, toys, or bowls. A towel or shirt with your scent is allowed. HappyTimes is not responsible for lost or damaged items. HappyTimes will provide all necessary items except for medications, treats, and food.
Feeding and Medication Instructions
Owner agrees to prepackage and label the dog's food for the entirety of their stay.
Medication
If the dog requires medication, the Owner must supply the medication with clear instructions for administration (including dosage, schedule, and method). We strongly encourage owners to bring medication in the original bottle with vet instructions and information included. HappyTimes will administer medication according to the provided instructions, but is not responsible for any adverse reactions or complications arising from the medication.
Special Care Requirements
If the dog has any specific care needs (such as separation anxiety, mobility issues, or other behavioral or medical conditions), the Owner should provide detailed instructions for HappyTimes to follow.
Dog’s Playtime and Interaction with Other Dogs
If group playtime or interaction with other dogs is available, Owner agrees to allow their dog to participate. HappyTimes will ensure that all participating dogs are properly supervised. However, Owner acknowledges that despite all safety measures, injuries or accidents can occur.
Emergency Transportation
If transportation services are needed (i.e. emergency vet), and are provided by HappyTimes, the Owner acknowledges that HappyTimes is not liable for any accidents or injuries that may occur during transport.
In case of an emergency, HappyTimes is authorized to transport the dog to the nearest veterinary clinic that is open and able to provide the necessary care to your dog.
Abandonment of Pet
If the Owner does not pick up the dog within 7 days of the agreed end date and no further communication or arrangements are made, HappyTimes reserves the right to consider the dog abandoned. The dog may be rehomed or transferred to a local animal shelter, and the Owner will still be liable for all costs incurred during this period.
Illness and Health Waiver
Owner understands that despite vaccination and health checks, there is always a risk of illness spreading in environments where multiple dogs are boarded. Owner agrees not to hold HappyTimes responsible for any illness contracted during or after the dog’s stay, as long as reasonable care and hygiene practices are followed.
Photo and Video Release (Optional)
Owner agrees to allow HappyTimes to take photos and videos of their dog during its stay, which may be used for marketing purposes, social media, or promotional materials.
Fees and Payment
Boarding Fees:
Owner agrees to pay a boarding fee of $45 per day for a suite or $36 per day for a crate (plus tax). Payment is due at the time of booking or upon pickup. Prices are subject to change, and HappyTimes will notify the owner of any changes before boarding.
Grooming Fees (if applicable):
Grooming services will be charged according to the selected service(s). A detailed grooming assessment and fee structure will be provided prior to service.
Late Pickup Fee
A $20 late fee will apply if the owner fails to pick up their dog within the agreed timeframe without prior notice. Owners must call ahead to inform staff if they are running late. No pickups are allowed after 7:00 PM. Dogs not picked up by 7:00PM will stay overnight, and the owner will be charged for an additional night of boarding.
Liability
The Owner acknowledges that while HappyTimes takes every precaution to provide a safe and caring environment, certain risks are inherent in dog care and boarding, such as illness or injury. The Owner agrees that HappyTimes will not be held responsible for any injury, loss, or damage to their dog beyond its reasonable control.
Owner Liability:
Owner assumes full responsibility for any damage or injury caused by their dog to other animals, persons, or property during the dog’s stay, whether at HappyTimes premises or elsewhere.
Termination
HappyTimes reserves the right to refuse or terminate boarding and/or grooming services at any time if it is determined that the dog poses a risk to the health or safety of staff, other animals, or itself.
Agreement Acknowledgment
By signing below, the Owner acknowledges that they have read, understood, and agree to all terms and conditions stated above.
